Passport applications at the Mission Viejo Library: appointments, documents and photos
The Mission Viejo Library accepts passport applications by appointment only. Appointment hours are Tuesday and Wednesday from 1:30 p.m. to 7:45 p.m., Thursday from 1:30 p.m. to 8 p.m., Friday from 1 p.m. to 4 p.m. and Saturday from 9 a.m. to 3 p.m. Time slots for the following week are posted on Fridays, and the library suggests checking the calendar often because cancelations open new appointments. The library's information line is 949-470-8420.
What to bring. The library lists these items for a passport application:
- Form DS-11, filled out before the appointment in black ink and not signed.
- Proof of citizenship, such as a certified U.S. birth certificate, a Certificate of Naturalization or a current or expired U.S. passport.
- Proof of identity, such as a valid driver's license, a valid passport book or card, or a military or government employee ID.
- Photocopies of the identification (front and back) and of the citizenship proof, in black and white.
- One color passport photo, 2 inches by 2 inches, taken within the last 6 months. The library offers a photo service for $15.
Children. All applicants must apply in person regardless of age. If the child is 15 or younger, both parents must be present and show acceptable identification. If one parent is unavailable, a notarized Form DS-3053 Statement of Consent from that parent can be used. Only one parent is required for an applicant who is 16 or 17.
Renewals. The library does not process Form DS-82, the self-service renewal form. Most renewals can be done by mail or online.
